Ontario Hepatitis C Teams Network

The Ontario Hepatitis C Teams provide prevention and care services for priority populations in Ontario who have challenging access to health care.

About the teams

The Ontario Hepatitis C Teams were created by the Ministry of Health to support marginalized populations in Ontario who have or are at risk for hepatitis C. The multidisciplinary teams provide "wrap-around care", which includes:

  • Clinical services such as testing, linkage-to-care and treatment.
  • Psycho-social support including counselling, mental health support, and referrals for housing, employment and other social services.
  • Community work such as outreach, education, prevention, harm reduction services and more.

The teams serve populations who face barriers to accessing mainstream health services. This includes:

  • People who use drugs
  • People who are homeless or underhoused
  • Youth who are street-involved
  • Indigenous people and communities
  • People involved in the provincial correctional system

The teams are grounded in principles of health equity, harm reduction and involvement of people with lived experience.
